A Life Care Plan is a dynamic document based upon published standards of practice, comprehensive assessment, data, analysis and research, which provides an organized concise plan for current and future needs with associated costs, for individuals who have experienced catastrophic injury or have chronic health care needs.

- LIFE CARE PLAN - A very comprehensive document, which address virtually all aspects of an individual's life. Areas addressed include: Projected Evaluation; Projected Therapeutic Modalities; Educational Needs; Wheelchair/Mobilities Needs and Accessories; Aids for Independent Function; Orthotics/Prosthetics/Home Furnishings; Medications and Supply Needs; Home Care/Attendant/Facility Care; Future Medical Care; Transportation Needs; Health Maintenance; Architectural Renovations; and Potential Complications.
- EVALUATION REPORT - A narrative report accompanies the Life Care Plan detailing the medical history; physical limitations; environmental influences; activities of daily living; education; employment history; recommendations; and summary of costs.
- MEDIAL DATA REVIEW - The development of an accurate understanding of the objective medical findings involved in each case requires a review and summary of all pertinent medical information. Emphasis is on the present disability, but also includes any related medical information.
- VOCATIONAL WORKSHEET - A Vocational Worksheet is attached to the Life Care Plan identifying the anticipated length of rehabilitation program; vocational handicaps; impact on placement; vocational development options pre-accident; and post-accident vocational alternatives.
- EXPERT TESTIMONY - Expert witness testimony provided by licensed, board certified consultants. All testimony is substantiated by publications, typically relied upon and utilized by other experts in the field.
